Before issuing a tax invoice
Use the supplier and purchaser details from their VAT registration certificates. Include only supplies subject to VAT and confirm the applicable rate. Keep invoice numbers unique within your own records.

Official format & sources
Based on Gazette 2481/22, Annexure I, with the effective date amended by Gazette 2500/106. The specimen permits layout changes that retain the required particulars.
